Jailers worry about dental floss as a weapon

(AP) ? Dental floss may prevent toothaches, but it's given jailers plenty of headaches.

When a group of New York prisoners sued last month to demand access to dental floss, officials said they had to consider "security issues." As it turns out, jail ? and jailbreak ? history is tightly tangled with the stringy decay fighter.

In Texas, officials believe a prisoner used floss to cut his way out of his cell, then jumped a fellow inmate and knifed him to death.

In Maryland, Illinois, West Virginia and Wisconsin, inmates collected enough floss to braid it into ropes and escape, or try to, over prison walls.

A group of escaped prisoners on the run in Texas used floss to sew up their gunshot wounds.

And a man in an Illinois jail used floss to stitch together the dummy he left in his bed when he took off.

Experts say floss, or the plastic holder it sometimes comes in, has been used to strangle enemies, to escape, to saw through bars, to pick handcuffs, to make a hand grip on a shank and to hoist contraband from one level of cells to another.

"These inmates can make a weapon out of a chewing gum wrapper," said Steven Kayser, whose company sells a floss product advertised as prison-safe. "Floss is right up there on the danger list."

Officials at the Westchester County Jail in Valhalla were somewhat leery when 11 inmates, acting without a lawyer, filed a $500 million lawsuit demanding access to dental floss.

Lead plaintiff Santiago Gomez said the jail was "violating inmates' federally protected civil rights by not allowing inmates access to dental floss, while acknowledging that it will result in cavities if you fail to floss your teeth."

He said the inmates had been brushing three times a day, "tongue and gums included," but were still getting cavities, bleeding gums, enduring constant tooth drilling and mental anguish.

Deputy Commissioner Justin Pruyne said the jail is not required to supply floss to inmates and said floss posed security concerns. But the jail has since brought in a supply of Kayser's "Floss Loops" ? circles of rubbery floss with no hard plastic that are designed to break easily before they can be used as a weapon.

It's not clear if that has satisfied the prisoners. The lawsuit has not been dropped.

An episode of the science TV show "Mythbusters" a few years ago set up an experiment to challenge the floss-as-security-risk theory. The show used a floss-equipped robot to test whether floss ? combined with toothpaste to make it more abrasive ? could really saw through a bar on a jail cell.

The feat was declared "plausible," given 300 days at eight hours a day ? the kind of time that an inmate might have.

Regulations of prison floss vary around the country.

In northeastern Texas, officials believe Antonio Lara used dental floss and toothpaste to saw his way out of a cell at a county jail and kill a gang rival in 2000.

"We do not carry traditional dental floss because of security concerns," said Jason Clark, spokesman for the Coffield Unit near Palestine, Texas. "Floss can be used to make ropes, weapons, cut through bars, even just reinforce the grip on a shank."

Instead, inmates are allowed plastic holders with a small piece of floss stretched across, he said.

In California, the state approves Floss Loops. Regular floss can be used as a garrote, said Terry Thornton, spokeswoman for the California Department of Corrections and Rehabilitation.

"But for that matter, you can pull thread from your socks or bed linens to choke someone. It's not just razor blades and toothbrush shanks in prison. We've seen underwear used as a weapon."

New York's state prison system permits dental floss, but only the unwaxed variety. The waxed is "much stronger," said spokesman Peter Cutler. He said prison security officials have banned it after having "experience" with the waxed variety. He would not elaborate.

The Westchester jail does ban toothpicks and water picks; inmates are allowed to have a 3-inch-long toothbrush, Pruyne said. Kayser said he also makes a "rubber-type" flexible toothbrush that's 4 ? inches long, but said anything longer could be fashioned into a weapon.

It was waxed floss that was used in Wisconsin in 1997 by inmates Timothy Dummer and Guy Dunwald. They used ropes of braided floss to get over a wall at the Green Bay Correctional Institution. They were quickly recaptured and had five years added to their sentences.

A television story about the episode said the prisoners had collected 18,975 feet ? more than 3? miles ? of dental floss.

In 1994, Robert Shepard used a floss rope braided as a telephone cord to scale an 18-foot wall at the South Central Regional Jail in West Virginia. He was on the lam for about five weeks. He was already being disciplined for scraping away the mortar between bricks in his cell.

In 1991, three inmates bought hundreds of yards of dental floss from the store at the Hays County Jail in Illinois and turned it into a ladder of sorts.

"It was ingenious," said U.S. Attorney Gerald Carruth when the men pleaded guilty to attempted escape. "They made the rope out of dental floss and used cardboard salt-and-pepper containers for stirrups on the ladder. ... That dental floss is strong."

Motorola Droid Razr Maxx HD (Verizon Wireless)

Wouldn't it be nice to just pluck your phone off the charger in the morning and not have to worry about it again until the end of the day? That's what you get with the Motorola Droid Razr Maxx HD, which turned in the best battery results we've ever seen on a smartphone. It also features a large 4.7-inch 720p display, along with a powerful dual-core processor and fast data speeds on Verizon's 4G LTE network. At $299.99, it's $100 more than the near-identical Droid Razr HD, but the Maxx gets you double the storage capacity and a few extra hours of battery life. If you need a phone you can use all day, it's worth it.

Editors' Note: The Motorola Droid Razr Maxx HD and the Motorola Droid Razr HD are extremely similar, so we're sharing a lot of material between our reviews. That said, we're testing each device separately, so read the review for your carrier of choice.

Size and Design
The Droid Razr Maxx HD looks almost identical to the Razr HD. Both phones measure 5.19 by 2.67 inches in height and width, but the Maxx is just a little thicker, at .37 inches compared with the Razr HD's .33 inches. It's also a little heavier, at 5.54 ounces compared with 5.15 for the Razr HD. To the naked eye, this difference is virtually imperceptible. Even when holding the phones it's hard to tell which one is which. So if you're worried about the Maxx being much larger, don't.

The phone is made using the same Kevlar material as the original Droid Razrs, which gives it an expensive, luxurious feel. Compared with the plastic?Samsung Galaxy S III?, the Razr Maxx HD feels much more solid. And it's protected by a water-repellant nanocoating, which doesn't make it waterproof, but a little more durable than the average smartphone.

The angular corners of the original Razr have been smoothed out for a sleek new look. Those colors only appear around the display; the back of each phone is the same gray and black patterned Kevlar, with an aluminum band running around the middle. There's a 3.5mm headphone jack up top, a Power button and volume rocker on the right, and a charging and micro HDMI port on the left, along with a hidden microSD card slot.

Design aside, keep in mind that this is still a very large phone. I have average size hands, and when I hold the Razr Maxx HD in one hand I can't reach high enough to drag the notifications bar down with my thumb. So if you have small hands or tiny pockets, you may want look at the 4.3-inch Droid Razr M, which is significantly smaller and easier to hold.

True to its name, the Razr Maxx HD is packing a 4.7-inch 1280-by-720 HD Super AMOLED display. It gets very bright and colors look super saturated, but I'm not a fan of the PenTile pixel layout, which, despite the 312 pixels per inch,?causes text and images to look a bit fuzzy upon close inspection. Function keys are displayed onscreen, and the large display means there's plenty of room for a big software keyboard that's very comfortable to type on.

Call Quality and Battery Life
The Razr Maxx HD works on Verizon's 3G and LTE networks, and can roam across the globe on GSM. It also integrates 802.11a/b/g/n Wi-Fi on the 2.4 and 5GHz bands.?Verizon's 4G LTE network is very fast, and received top honors in our?Fastest Mobile Networks?tests earlier this year. The Razr Maxx HD has good reception, and averaged 5.5Mbps down and 6Mbps up in our speed tests.

Call quality is good. In my tests, voices sounded rich, though a little bit muddy, in the phone's earpiece. I do wish the volume level went just a little bit higher; it was difficult to hear everything outside on a busy city street. Transmissions sound clear, with good noise cancellation. Calls sounded fine through a?Jawbone Era?Bluetooth headset and Motorola's voice command system worked well over Bluetooth. The speakerphone also sounds fine, but isn't loud enough to use outdoors.

The most important feature here is the battery, and to that end, the Razr Maxx HD delivers. The nonremovable 3,300mAh battery was good for 18 hours and 51 minutes of talk time, which is the best result we've ever seen from a smartphone. Compare that with the Razr HD, which got 14 hours and 48 minutes, and the Galaxy S III, which was good for 10 hours and 43 minutes, and the Razr Maxx HD is well ahead of the competition.

We also tested battery life while streaming a video over 4G. The Razr Maxx HD lasted for just about 9 hours, with the Razr HD coming in at 7 hours and 1 minute, and the Galaxy S III at 6 hours and 57 minutes. All of these results are respectable, but if you need a phone that's guaranteed to make it through the day, the numbers speak for themselves.

Processor, Android, and Apps
Powered by a 1.5GHz dual-core Qualcomm Snapdragon S4 MSM8960 processor, the Razr Maxx HD is built around the same processor you'll find in the Razr M and the Razr HD. It's starting to become somewhat standard among the upper echelon of smartphones, but that doesn't make it any less powerful. The Razr Maxx HD turned in similar benchmarks to the Galaxy S III, which is powered by the same chip. It's powerful enough for any apps or games you throw at it, and should keep things moving at a steady clip for some time to come. Keep in mind that gaming frame rates are actually higher on the Razr M, because its lower-resolution display means it's pushing fewer pixels.

The Razr Maxx HD ships running Android 4.0.4 "Ice Cream Sandwich," which is nearly a year old at this point. That's kind of crazy considering Google now owns Motorola, and Android 4.1 "Jelly Bean" brings a host of improvements over ICS. An update is planned before the end of the year, but it should be there to begin with.

HBT: Rivera 'having 2nd thoughts' about playing again

While acknowledging it?s still ?probable? that Mariano Rivera will return from knee surgery to pitch next season, Andrew Marchand of ESPN New York reports that the future Hall of Famer ?is having second thoughts about possibly retiring.?

Rivera was pretty adamant about returning at the time of the injury, saying: ?I?m coming back. Put it down. Write it down in big letters. I ain?t going down like this.?

However, according to Marchand?s unnamed sources Rivera ?has the familiar tug that he has carried for many years of possibly wanting to stay home with his family and call it a career.?

Rivera is a free agent, so if he does want to play in 2013 he?ll presumably have to let the Yankees know fairly soon so they can plan accordingly and, eventually, come to terms with the 42-year-old on a new contract after he earned $15 million this season.

Which One Of These Do I Have To Spill Water On To Make People Irrationally Start Buying $FB

Screen Shot 2012-10-22 at 8.02.55 PMThose eighteen words got a founder and I kicked off of the trading floor of the NYSE earlier this evening. Because, as we shuffed out of the Girls Who Code event held there earlier tonight, we looked at the jumble of computers and stock tickers on the first floor of 18 Bond Street and exclaimed in awe, “So this is where it all goes down, huh?” And of course, because we’re tech people and troublemakers we’re like, “So how does it work?” specifically joking, in light of all the recent tech stock collapses, “Which one of these do I have to spill water on (okay, we may have said wine) to make people irrationally buy Facebook?” The founder and I (who made me promise not to write about him/her because he/she said his/her?parents PR people would kill him/her and I don’t want to be an accomplice to murder) took solace in this laughter, and the idea that the rise in price of $FB shares would most likely bolster the entire tech industry. And all was bright and shiny until an NYSE security guard pulled the founder aside from our revelry. “Excuse me sir/mam, did you just ask which computer you’d have to spill water (okay, it was most likely wine) on to make people buy Facebook stock?” Oh God. “We were joking,” I replied, which, like how you can’t say bomb?in any context on a plane, apparently isn’t acceptable in the NYSE. Because we got kicked out. “Get OUT” the guard said, and we scurried out, because he really did mean it. One man’s facetiousness is another man’s terrorist threat. And yes, I have the utmost respect for this guard. Because this exactly was his job. So we got OUT, like ran out, before the man could take further action, and I hope we’re not banned for life because I have to be at an another event at the NYSE on Thursday. Also, Girls Who Code is awesome, and you should all download 16-year-old “Girl Who Codes” Nikita Rau’s app Wander. And not joke about destroying important things, ever. Pro tip.

How to Generate Blog Traffic Using Reddit | Geek Business

Blogs inhabit a special place in the world of search engine optimization. Featuring a special niche and topic, blogs are a salient source of selective information. Though they are reliable source of knowledge and information, their presence on the web is only appreciated by the amount of traffic it receives from the Internet community. The traffic a blog receives depends on a number of factors like quality & uniqueness of the content, SEO and social media influence. Of all the factors, the influence of social media plays a vital role in publicizing a blog to the online community. The prominence of social media and its influence in advertising and marketing has become quintessential.

Social networking, social bookmarking and micro-blogging are some of the famous social media platforms. There are plentiful websites in each of these domains that serve their purpose substantially. One such popular social news website is Reddit that lets registered users submit content. Launched in 2005, Reddit is a multilingual social news website that serves wide spectrum of online communities. The website accumulates the content submitted by registered users and displays them using a bulletin board system. Registered users can rank the submitted content by voting ?up? or ?down? for the content. Depending on the number of votes, the content will be displayed in the front page of the Reddit website.

The contents are organized under categories called subreddits with certain of them being set as default. It is estimated that there are more than 67,000 subreddits as of May 2012. For all those zealous bloggers who would like to monetize their blog, it is important to find out new ways to appeal to new audiences. Reddit helps bloggers appeal to new audiences and also get the all-important online traffic.

Being the part of the Reddit community will help you harness the power of social media and sharing websites to promote your blog. The one action that is prohibited in the Reddit community is spamming and users engaging in such activities are severely dealt with. While promoting your blog, it is also important to note that submit only your own content to Reddit is considered as gaming the system, so spend some time posting other interesting content with the necessary credits. I have elaborated below things to keep in mind before submitting content to Reddit.

  • Submit content that will attract your target audience, while also keeping in mind the aspects of a regular Redditor.
  • Present your content with interesting titles and images.
  • Submit your content under the suitable subreddit.
  • Finally, actively engage with the Reddit community.

Success on this social news website depends on your niche and the presentation of your content. And eventually when you succeed on Reddit, you are certain to see increased page views and unique visitors.

Live Review: Kishi Bashi and Tinariwen | Triangle Arts and ...

Violinist, beatboxer, and electronic one-man-indie-band Kishi Bashi, introduced Tinariwen last night at The ArtsCenter in a performance that could have easily been a stand-alone headliner.? Gradually recording and layering melodic violin loops, ranging vocals, percussion sounds, and synth effects, he constructs his beautiful songs piece by piece in front of the audience.? This process allows the listener to appreciate every part of the song individually and evokes curiosity and anticipation as to where the song will lead, what it will eventually become.? What emerges harmoniously from the broken music is impassioned indie pop with the depth of a symphony.

As a gifted violinist, Kishi Bashi previously recorded and toured with Of Montreal and Regina Spektor. The audience?s cheers and enthusiasm were a testament to his growing popularity as he played songs from his 2011 EP Room for Dream and his recent 2012 album 151a.? Recognized as one of the best new artists of 2012 by NPR?s All Songs Considered, Kishi Bashi revealed a creative progression of sound that that amazed fans and first-time listeners alike during his live solo act.

When Grammy-award winning ?soldier musicians? Tinariwen took the main stage at The ArtCenter following Kishi Bashi, both group and audience got their Sahara Desert blues groove on.? Since the 1980s, the Toureg rebels of Tinariwen have used music as their way to speak against the injustices of their nomadic people, forced into sedentary life, and the unforgiving nature of the desert.

Electric and acoustic guitars, simple percussion including a calabash drum, soft vocals, and an occasional shepherd?s flute create their unique rock and roll sound.? Although their music has roots in the West African tradition, the members of Tinariwen (who have changed throughout the years) were influenced by Western icons like Jimi Hendrix, Santana, and Led Zeppelin.

The fusion of musical styles was apparent during their performance, which was full of funky bass lines and electric guitar riffs. Much of the crowd left their seats after the first couple of songs and migrated toward the front of the stage to dance throughout the night to the compelling rhythms. The musicians? smiles, dancing, and upbeat guitar-driven music encouraged the crowd to participate.? Tinariwen recorded their most recent album Tassili isolated in the southern Sahara Desert around a campfire.? Listening to one of the slow, mournful ballads you could almost imagine being part of that communion in the intimate atmosphere of the theater.

Skipping Breakfast Makes Us Hunt For Fat

Fruit in a glassThese days I love a nice green juice or smoothie for breakfast, yet when I was over weight I would often skip breakfast altogether. Brain scans show that skipping breakfast makes fatty, high calorie foods appear far more attractive later in the day, according to researchers.

Scans of 21 people showed the brain was more attracted to food if breakfast was missed so people had more food at lunch.

Scientists said it made losing weight challenging as missing meals made calorific food even more appealing.

Nutrition experts say breakfast is known to take the edge off appetite.

However, researchers were curious about what happened inside the brain to alter the food people choose to eat.

Twenty one people, who were all normal weight, were shown pictures of calorie packed foods while they were positioned in a magnetic resonance imaging (MRI) machine at Imperial College London.

On one day they were given no breakfast before the scans and on a different day they were fed a large, 730 calorie, breakfast an hour and a half before.

The researchers said skipping breakfast created a ?bias? in the brain in favour of high calorie foods.

The results, presented at the Neuroscience 2012 conference, showed the brain changed how it responded to pictures of high calorie foods, but not low calorie foods, when breakfast was skipped.

They showed part of the brain thought to be involved in ?food appeal?, the orbitofrontal cortex, became more active on an empty stomach.

When the researchers offered the participants lunch at the end of the study, people ate a fifth more calories if breakfast was missed.

Dr Tony Goldstone, from Imperial College London, said: ?Through both the participants? MRI results and observations of how much they ate at lunch, we found ample evidence that fasting made people hungrier, and increased the appeal of high calorie foods and the amount people ate.

?One reason it is so difficult to lose weight is because the appeal of high calorie food goes up.?

Dr Catherine Hankey, a senior lecturer in nutrition at the University of Glasgow, said research had shown that breakfast ?takes the edge off appetite? and that the latest study was an ?interesting? insight.

She said breakfast was linked to stable blood sugar levels, which ?keeps you on the straight and narrow?.

Future studies will investigate how obesity affects the same system in the brain.

7 Smart Tips on Staying Focused

staying focused

Staying focused on my work has been of the biggest challenges that I had to overcome. It is so easy to get distracted with the information overload that we are bombarded with every single day.

From emails, Facebook, twitter, enviously reading our competitors websites and taking on more than we can handle, I knew I had to get a grip and become highly focused on only the work that produced results.

Here are my top 7 Tips for straying focused.

1.??????????? Do not check email until at least 4pm. Email can ruin you. Learn to use it to your advantage and do not let it use you.

Opening your email first thing in the morning will set you down a massive rabbit hole of being driven by other people?s demands and take you off your primary focus ? building your business and achieving your goals.

?

2.??????????? Only ONE hour of power of Facebook. Before you begin your Hour of Facebook Power ? use this checklist:

?

  • What is my goal ? what do I want to achieve from getting on Facebook. It may be to make new connections, to get information, to help others, to look for potential leads and clients or simply to connect with your friends. It doesn?t really matter what is it ? as long as you are CLEAR on the goal and you stick to it
  • Set a timer and do not go over the one hour. Remember, there is always tomorrow.
  • Stay on Facebook. It is Facebook time, so do not go on any other site during this time. You only have one hour so make the most of it.

?

3. Make a list of the 3 most important things that you need to do each day to move your business forward. Keep this list where you can see it constantly and whenever you are doing something ask yourself the question: ?Is this going to help me grow and move my business forward??

?

4. Make a list of all the things that you must stop doing immediately in order to stay focused. This can mean:

  • Putting your phone on silent and putting it in another room,
  • ?Not going onto other people?s websites,
  • Eliminating multi tasking,
  • Avoiding conversations ? both online and offline that drain your time and energy.

?

Make your own list and post it where you can see it. Not doing certain things that you have identified as a major bottleneck to your focus and productivity is equally as important as doing your 3 most important things that move your business forward.

?

Be ruthless and eliminate the desire to apologize, feel guilty and explain yourself to people. Those who matter don?t mind, those who mind don?t matter.

?

?

5. Take regular breaks ? this could be going to a yoga class, going for a walk, a swim, or some simple guided meditations. This is usually when your best ideas will come and will actually keep you focused because they will keep your body, mind and spirit invigorated and ready to take more focused action

?

6. Plan out your day and be ruthless about sticking to your plan. Do not let anything distract you. Put your phone away, shut down any sites that are distracting and train others by letting them know the times that you are not available.

?

If they don?t like it, that is none of your business, if you lose friends, they probably were not worth keeping in the first place. Better yet, you may teach them a lot about boundaries and how to stay focused and achieve their own goals.

?

7. Create a clutter free environment. In order to stay focused and work with clarity, your environment needs to be supportive of your intention. Clear away any visible distractions. Create empty, white space where you work and practice maintaining a minimalist work environment.

How do you stay focused on your work and your major goals?

After a few laughs, Romney, Obama back to the fray

NEW YORK (AP) ? President Barack Obama and Republican challenger Mitt Romney are returning to the sometimes-nasty rhetoric of a close presidential campaign after a brief truce, renewing their focus on two battleground states and preparing for next week's final, perhaps pivotal, debate.

Romney and Obama set aside their differences ? mostly ? to poke fun of themselves and each other Thursday night at the Alfred E. Smith Memorial Foundation Dinner. On Friday, it's back to campaigning in Florida and Virginia, two of just a handful of states that will decide the election, now less than three weeks away.

Obama was planning a speech at George Mason University in Fairfax, Va., rallying college students in the northern part of the state. Romney was to fly to Daytona Beach, Fla., for a rally with running mate Paul Ryan.

While they're both focused on the South, an NBC News/Wall Street Journal/Marist polls released Thursday showed Obama retaining his lead over Romney in Iowa and Wisconsin, two Midwestern battlegrounds. Obama's campaign circulated a memo highlighting the president's strength during the early voting period in Ohio, where Romney has largely staked his hopes of winning the White House.

But both kept relatively light public schedules as they planned to spend the weekend preparing for the third and final presidential debate, set for Monday in Boca Raton, Fla. Obama was leaving for the Camp David presidential retreat to prepare, while Romney planned preparations in Delray Beach, Fla.

"The next debate is on foreign policy," Obama told the white-tie audience at the Thursday evening dinner. "Spoiler alert: We got bin Laden."

Authorizing the raid that killed the terrorist leader was a high point in Obama's first term, and polls show voters give him high marks on handling foreign policy. But the attack on the U.S. Consulate in Benghazi, Libya, last month has prompted Romney and Republicans to raise questions about Obama's policy in the Middle East.

In a television interview Thursday, Obama rejected criticism that his administration has offered a confused response to the attack in Benghazi, an accusation Romney has made repeatedly. Of any breakdown that might have led to the killing of four Americans, Obama declared, "We're going to fix it."

Obama made the comments on "The Daily Show" on Comedy Central. Host Jon Stewart pressed Obama over his administration's shifting explanations about the attacks in Benghazi. When Stewart suggested that even Obama would concede his administration's coordination and communication had not been "optimal," Obama said: "If four Americans get killed, it's not optimal. We're going to fix it. All of it."

Romney has pointedly questioned Obama's handling of the matter and how honest he's been about it with Americans. Those accusations led to the fiercest conflict of the presidential debate on Tuesday and surely will come to the fore again Monday in the campaign's final debate.

Obama insisted information was shared with the American people as it came in. The attack is under investigation, Obama said, and "the picture eventually gets filled in."

After that interview, Obama headed to the Waldorf Astoria hotel, where he sat one seat over from Romney at the charity dinner, an annual gala that has drawn political leaders and other notables since the end of World War II. The event was a comedic pause in a contest that has drawn increasingly nasty and close.

Romney spoke first at the dinner, which was set to raise $5 million for Catholic charities. Addressing the elegantly dressed crowd, Romney, a millionaire many times over, said, "It's nice to finally relax and wear what Ann and I wear around the house." Of Obama, Romney said: "You have to wonder what he's thinking. So little time, so much to redistribute."

Obama followed, noting his soporific performance in the first debate but also chiding Romney for his wealth.

"Earlier today I went shopping at some stores in Midtown," Obama said. "I understand Governor Romney went shopping for some stores in Midtown."

He looked to his left, where Romney sat, grinning as his opponent laughed.

Elevated risk of blood clots in women taking birth control containing drospirenone, study shows

ScienceDaily (Oct. 18, 2012) ? A U.S. Food and Drug Administration-funded study led by the Kaiser Permanente Northern California Division of Research found an increased risk of arterial thrombotic events (ATE) and venous thromboembolic events (VTE) -- commonly referred to as blockage of arteries and blood clots, respectively -- associated with drospirenone-containing birth control pills compared to four low-dose estrogen combined hormonal contraceptives.

The study appears in the current online issue of Contraception.

"We found that starting use of drospirenone-containing combined hormonal contraceptives was associated with a 77 percent increase in the risk of hospitalization for VTE and doubling of the risk for ATE compared to the risk associated with use of the four low-dose estrogen CHCs," said Stephen Sidney, MD, MPH, director of Research Clinics at the Kaiser Permanente Northern California Division of Research and lead author of the study. "Though the absolute incidence of venous thromboembolic events is low, the growing number of studies showing an increased risk of venous thromboembolic events with drospirenone suggests that drospirenone-containing combined hormonal contraceptives should be used cautiously for women seeking hormonal contraception. We also need more information on the risk of arterial thrombotic events with drospirenone-containing combined hormonal contraceptives because few data have been published."

"To put these numbers into perspective, the risk of developing blood clots is higher when using any birth control pills than not using them, but still remains lower than the risk of developing blood clots in pregnancy and in the postpartum period," said Dr. Sidney. "Nonetheless, health care professionals should consider the risks and benefits of drospirenone-containing birth control pills and a woman's risk for developing a blood clot before prescribing these drugs."

Drospirenone (DRSP) is a synthetic version of the female hormone, progesterone, also referred to as a progestin. During the past 10 years, three new combined hormonal contraceptives (CHC) preparations have been approved for use by the FDA, including drospirenone/ethinyl estradiol pills (DRSP), the norelgestromin /ethinyl estradiol transdermal patch (NGMN), and the etonogestrel/ethinyl estradiol vaginal ring (ETON). Since then, there have been several studies evaluating the risk of thrombotic and thromboembolic events compared to low-dose estrogen CHCs that have been on the market for longer periods of time.

"The results have been mixed, and it is unclear whether the differences in findings arose from differences in study methodologies or differences in the populations studied. As a result, there is a great deal of concern and confusion among women and their health care providers regarding the safety of these newer preparations relative to older CHCs," Dr. Sidney said.

"We performed this retrospective cohort study to address methodological issues using a 'New User' design. We assessed the risk of each of the three newer CHCs relative to low-dose estrogen CHCs in a cohort of new users of CHCs from four geographically and demographically diverse health plans."

The study cohort consisted of 573,680 women, ages 10-55, who were identified as "new users," which was defined as first exposure to any of the three DRSP-containing CHCs or the four low-dose estrogen CHCs during the 2001-2007 study period. The cohort was drawn from two integrated health care programs (Kaiser Permanente Northern California and Kaiser Permanente Southern California) and two state Medicaid programs (Tennessee and Washington).

"While the absolute risks for venous thromboembolic events in patients taking drospirenone-containing birth control pills is low, this research helps inform the conversation health care providers have with patients about their oral contraceptive options," said Tracy Flanagan, MD, director of Women's Health at Kaiser Permanente Northern California. "We offer many oral contraceptive choices and can offer alternatives to drospirenone-containing pills that may be more appropriate for an individual patient. The risks and benefits of any contraceptive should be weighed carefully against the risk of an unintended pregnancy."

The comparator CHCs included levonorgestrel/ethinyl estradiol tablets (LNG10-20), levonorgestrel/ethinyl estradiol tablets (LNG15-30), norethindrone/ethinyl estradiol tablets (NETA), and norgestimate/ethinyl estradiol tablets [NGM].

New users of DRSP had 1.77 times the risk for VTE and 2.01 times the risk of ATE relative to new users of the low-dose estrogen comparators. The increased risk of DRSP was limited to the 10-34 years age group for VTE and 35-55 years group for ATE.

Use of the NGMN patch and ETON vaginal ring were not associated with increased risk of either thromboembolic or thrombotic outcomes. However, the patch and ring are used by far fewer women than DRSP-containing birth control pills.

In a Drug Safety Communication dated April 10, 2012, the FDA wrote that based on its review of observational studies, it has concluded that "drospirenone-containing birth control pills may be associated with a higher risk for blood clots than other progestin-containing pills." The FDA advises that, even though it is unclear whether the increased risk seen for blood clots in some of the epidemiologic studies is actually due to drospirenone-containing birth control pills, women should talk to their health care professional about their risk for blood clots before deciding which birth control method to use.

Funding for the study was provided by the U.S. Food and Drug Administration Office of Surveillance and Epidemiology, Center for Drug Evaluation and Research.

Additional authors on the study included T. Craig Cheetham, PharmD, MS, with Pharmacy Analytical Services, Kaiser Permanente Southern California; Frederick A. Connell MD, MPH, with the University of Washington School of Public Health; Rita Ouellet-Hellstrom, PhD, MPH, David J. Top security official killed in Lebanon car bombing

BEIRUT (AP) ? Lebanon's state-run media says a senior security official was killed in Friday's car bomb attack in Beirut.

The National News Agency said in a statement that it "announces the martyrdom" of Maj. Gen. Wissam al-Hassan, the head of the national police intelligence unit, in the explosion that ripped through east Beirut.

Al-Hassan was among at least eight people killed. The bomb wounded dozens of other, sheared the balconies off apartment buildings and sent bloodied residents staggering into the streets in the most serious blast the Lebanese capital has seen in more than four years.

Type M for Murder: Musings From a Scattered Week

A lot going on this week, but all of it seems disjointed.

Tuesday, I ran a short-story-writing workshop in my daughter's sixth-grade class. It was the culmination of my visit two weeks earlier when I helped each student create a character sketch and write a compelling opening line. They went home and drafted a story. This week, I returned with five high school students from my creative writing workshop, and we helped the students revise. My daughter's teacher will take it from here, and they are to produce a literary journal with all the stories in it. I can't wait for my copy!

On the publishing front, several blurbs for my 2013 novel "This One Day" have rolled in, which is exciting and makes it feel like having the hardcover in my hands is actually inevitable.

The rest of the week has been spent working on deadline. I've been up early to work a couple hours before school and staying up late to work an hour or so before bed (candle at both ends, so to speak).

I attended a round-robin poetry reading Tuesday. The format consisted of four poets sitting in a row, each reading a poem that related to (or attempted to relate to) the previous poet's poem. Very interesting. Very funny. How neat would that be to do with fiction writers, especially crime writers. Read a scene and have someone follow it with a scene of their own. Anyone ever hear of such a thing?

Common Cold Virus Could Be Turned Into Anti-Cancer Drug ...

I think we can agree that the common cold sucks. I also think we can agree that it doesn?t suck nearly as badly as cancer. Researchers at the Salk Institute, though, may be on the trail of a way to turn the lesser of these two evils into a weapon in the fight against the other.?This week, a study in the journal Cell reports that the Salk team has taken steps toward hijacking the common cold?s ability to disable immune responses within cells. That could lead to engineered cold viruses that hunt down and destroy cancer cells, while leaving healthy cells in peace.

When you get sick with a common cold, it?s because some of the countless cells in your body have been hijacked by the virus. In many ailments, that triggers genes active within the cell to issue a self-destruct command, because cells would rather sacrifice themselves than continue pumping out new copies of the cold virus to infect other cells. The common cold virus, adenovirus, can disable this command using a protein that suppresses the cells? suicide genes, preventing infected cells from hitting the big red button and keeping them functioning as virus factories. That means a second wave of immune cells has to be dispatched to eliminate the hijacked cells, and you?re going to have a plugged nose and a scratchy throat for the whole ordeal.

The study released this week details new findings on the structure of the protein ??E4-ORF3 ? that adenovirus uses to keep cells from self-destructing and taking one for the team. The protein forms a molecular net around p53, a protein produced in healthy cells that activates the genes responsible for the cell wiping itself out. That quality also makes p53 a tumor suppressor, and when it shuts down, cancer can follow ? p53 action is suppressed in almost all cancer cells, letting tumors spread through cells incapable of destroying themselves.

Understanding how cold viruses hijack our healthy cells is important in its own right, but this line of research could one day lead to engineered viruses that can be unleashed on tumors like viral attack dogs. The long term goal would be a line of viruses that could only make copies of themselves in cells where p53 is suppressed already ? a self-perpetuating virus that only attacks cancer cells. That?s a way off ? taking out just enough teeth to make adenovirus dangerous to tumor cells and harmless to normal ones is no small task ? but the prospect of turning two diseases on each other is just too cool not to get at least a little bit excited about.
